Photo and video consent form

For academies, clubs, schools and events: which uses of photos and videos each person allows, with their signature.

Ask for permission before you publish photos or videos: the person says whether they allow the use of their image, ticks what for (website, social media, printed material or internal use) and signs. For a minor, their parent or guardian signs.

Each consent is stored with its date and signature, and you can export them to CSV or print each one. Review the data protection information and add your name before publishing.

Free. You can change every question.

The data it asks for

Consent must be specific: that is why the person ticks each use they allow and can say no. For a minor, the template asks for their parent's or guardian's details. Each form deletes its responses, and their files, once the retention period you choose has passed (6 months by default). For consent that must last as long as you use the images, choose a long period or keep the PDF.

Frequently asked questions

Is the signature valid?
It is a simple electronic signature: it records who accepts, when and with which details, and it cannot be denied effect just because it is electronic. It is not a qualified electronic signature; if a specific procedure requires one, a digital certificate is needed.
Who signs for a minor?
When the person is marked as a minor, a question for their parent or legal guardian appears. The age at which minors can consent themselves depends on the country, so check the rules that apply to you.
What if someone changes their mind?
They can withdraw their consent at any time. Record the change and stop using the images for the purposes they withdrew.
